Telehealth Informed Consent

This consent describes the nature, benefits, and limits of telehealth services provided by Seavaint Health Nursing Inc, dba Seavaint Health. By using our telehealth services, you acknowledge that you have read and understand this consent.

1. What telehealth is

Telehealth involves the use of secure electronic communications, including audio and video, to provide healthcare from a distance. Your clinician may evaluate symptoms, review history, recommend treatment, prescribe medication when clinically appropriate, and order labs.

2. Benefits

Telehealth can improve access to care, reduce travel time, and offer convenient follow-up.

3. Limits and risks

  • Telehealth may not be appropriate for every condition. Some symptoms require in-person evaluation, lab work, or physical examination.
  • Technology issues, including poor connection, may disrupt or end a visit.
  • A clinician may determine that telehealth is not safe or appropriate for your situation and direct you to in-person care.
  • Some prescriptions may have added requirements, including controlled substances, which may require in-person visits or additional documentation.

4. Not for emergencies

Telehealth is not appropriate for medical or mental health emergencies. If you are experiencing an emergency, call 911 or go to the nearest emergency room. For mental health crises, call or text 988.

5. Privacy and confidentiality

Telehealth visits use HIPAA-aware platforms. We take reasonable measures to protect the privacy of your visit, but no electronic communication is completely secure. You can help by joining visits from a private place and using a secure internet connection.

6. California-only service

You must be physically located in California at the time of your visit. Visits with patients outside California are not permitted under our practice scope.

7. Records

Information from your telehealth visit becomes part of your medical record and is governed by our Notice of Privacy Practices.

8. Right to withdraw

You may withdraw consent for telehealth at any time. Withdrawal will not affect the quality of care you receive going forward.

9. Acknowledgment

By proceeding with a telehealth visit, you confirm that you have read this consent, had an opportunity to ask questions, and understand the nature, benefits, and limits of telehealth.
